Request Technical Service

STEP 1 - PREPARE REQUIREMENTS. To enable us to help you with your request of technical service must provide the items listed:

  1. Technical Service Request Form
  2. Copy of valid Government issued ID
  3. Proof of Purchase and Ownership
    1. For Private buyers, sole proprietors:
      1. Proof of Purchase (e.g. Official Receipt/Sales Inv./Delivery Receipt) OR
      2. Notarized Affidavit of Purchase and Ownership*

      *For Force Open requests, the Notarized Affidavit of Purchase and Ownership is required.

    2. For Corporations/Partnerships:
      1. Secretary’s Certificate OR
      2. Authorization Letter by Officer of the company (as listed in the GIS) with an
        attached copy of the company GIS
  4. Payment for service 
    If the product is under warranty and ZMC, in its sole opinion, has determined that the problem
    is a manufacturer warranty issue, inspection fee and repair costs will be waived.

STEP 2 - SUBMIT all completed forms to us via one of the following methods:

Notes: 
       1) All notarized documents must be dated within 30 days of initial submission to ZMC.
       2) Zenorex Marketing Corporation services only products sold by ZMC, ZMC retail 
           partners or non-competitors (e.g. SENTRY USA).
