Request Combination Code and/or Duplicate Key

Step 1 -    To enable us ­to help you with the combination code or a duplicate 
                 
key you must provide the items listed:

Case 1 – Original Owner
          1.1   Letter of Request - indicate the model number, serial number and place & date of purchase.
                  If requested by a company, it should be on company letterhead.
          1.2   Photocopy of valid I.D.
          1.3   Proof of purchase (i.e.Official Receipt showing owner’s name) or 
                  affidavit of ownership – notarized*download-notary.png
          1.4   Payment for code retrieval / key replacement

 

Case 2 – In case of death of the original owner
          2.1   Letter of request from heir (ex. wife, husband, children etc.) - indicate the model number
                  and serial number.
          2.2   Affidavit of ownership – notarized*download-notary.png
          2.3   Photocopy of valid I.D.
          2.4   Death certificate
                  2.4.1   If husband or wife - marriage contract
                  2.4.2   If child - birth certificate
                  2.4.3   If brother or sister – birth certificate of both

          2.5   Payment for code retrieval / key replacement

 

Case 3 – If given as a gift or second owner
          3.1   Letter of request from present owner or letter of authority from the original owner -
                  indicate the model number and serial number.
          3.2   Photocopy of valid I.D.
          3.3   Affidavit of ownership – stating how you acquired the unit (notarized*)download-notary.png
          3.4   Payment for code retrieval / key replacement

 

Case 4 – If purchased from a non-competitor of ZMC (e.g. SENTRY USA)
         4.1   Letter of request from the owner - indicate the model number and serial number
         4.2   Photocopy of valid I.D.
         4.3   Affidavit of ownership – stating how you acquired the unit(notarized*)download-notary.png
         4.4   Payment for code retrieval / key replacement
